Chief Minister’s Relief Fund Process
- Eligibility Check
- The patient must have been diagnosed with cancer to be eligible for the relief fund.
- The CMRF typically provides financial aid to individuals who are financially unable to afford major health treatments, including cancer.
- The patient's Ayushmaan card will be checked through the Samagra ID portal. If the Ayushmaan card is valid, the treatment will be conducted through the Ayushmaan Bharat scheme and not the Chief Minister's Relief Fund.
- Gather Necessary Documents
- Application form which can be obtained from the hospital
- Detailed medical reports, including diagnosis reports like Biopsy, etc and treatment plans including Chemotherapy, Radiotherapy and Surgery
- Estimate of the treatment cost from the hospital (The treatment cost estimate will be provided according to the guidelines of the State Government Health Department).
- Identity proof (Aadhaar card), Samagra ID and a Photograph
- Submission of Application
- Submit the application along with all the required documents through a Member of the Legislative Assembly (MLA) from the constituency where the applicant or patient resides. The MLA will then forward it to the Chief Minister's Office (CMO).
- Verification Process
- Once the application is received, it will undergo a verification process. The CMO may contact the hospital and the applicant to verify the details provided.
- If the application is approved or rejected, the patient will be informed through SMS.
Important Points
- Ensure that all documents are genuine and complete to avoid delays or rejection.
- Maintain a copy of all documents submitted for your records.
- The Chief Minister’s Relief Fund can be utilised only once per financial year.
- The treatment cost estimate will be provided according to the guidelines of the State Government Health Department.
- A second estimate for treatment can be approved only if the amount of the first estimate has been completely utilized.
